Hospice
When patients are faced with a life-limiting illness, the focus shifts from curing to caring through hospice, also known as end-of-life or comfort care. When a physician determines a patient may have a life expectancy of six months or less, trained hospice nurses provide a specialized type of medical care focused on symptom management and quality of life. In addition to physical, emotional, and spiritual support, Lifesong’s complementary therapies such as, music therapy and/or pet therapy may also be included in the care plan. Patients are celebrated through the Legacy Program and compassionate and dedicated volunteers provide support to patients and their families throughout their journey.
Care Process
Within the first five days, the care team including a physician, nurses, a social worker, a chaplain and a hospice aid are assigned to provide care aligned with the personalized care plan discussed during the assessment. This allows family and friends to enjoy quality time with their loved one.
Palliative Care
Palliative care is coordinated for patients at any stage of a seniors illness and can be delivered alongside curative treatments. It focuses on managing the physical and emotional impacts of the illness while also offering support and guidance to help patients and families navigate their care options.
Bereavement Services
Grief and loss after the death of a loved one can feel overwhelming and no one is ever fully prepared for it. Everyone experiences it differently, and the emotions that follow are deeply personal. There can be a mix of many emotions, including sorrow, confusion, fear and anger. The Lifesong care team is here to walk alongside through the grief journey with compassionate support through:

- Memorial services focused on the meaning, purpose and impact of the loved one’s life
- Resources

Veterans Services
Lifesong acknowledges a patient’s military service as a way to honor their sacrifice, recognize their unique lived experiences and deliver care that respects theirmilitary contributions. Our care teams are trained to understand Veteran culture and organize events and tributes intended for veteran patients and their families.
