Our Difference

It's a Privilege to Care for Your Family

When faced with hospice and palliative care decisions, Lifesong understands that patients and their families are navigating more than a diagnosis. It’s about making their loved one comfortable while celebrating a full life.

We know and value family, because we are a family.

Today the Leaders, the family of former Pennsylvania Governor George M. and Mary Jane Leader, continue the legacy of providing quality care and support for seniors at every step of the care continuum. From independent living, personal care and memory support services at Country Meadows Retirement Communities, Ecumenical Retirement Community and Providence Place Senior Living to supporting patients and their families nearing the end of their journey with hospice and palliative care through Lifesong, the family has been providing compassionate patient/resident-centered approach in Pennsylvania for more than 40 years.

Lifesong Hospice and Palliative Care serves patients residing in their homes or other retirement, assisted living or skilled nursing communities throughout Central Pennsylvania and the Lehigh Valley. Lifesong services are not exclusive to supporting residents living in Country Meadows-affiliated retirement communities.

Care Philosophy

At Lifesong, we seek to individualize our hospice and palliative care to help make the final journey as comfortable as possible and tailored to your loved one’s needs and preferences by:

  • Providing quality and customized care
  • Celebrating the meaning of their lives and legacy
  • Maximizing quality of life and preserving dignity

Delivering Quality Care

Care is provided wherever the patient calls home. Whether in a senior living community, a hospital or rehabilitation center or in the comfort of the family home, our licensed nurses, social workers and our own Medical Director, Dr. Joan Harrold, provide timely consultation and symptom management through developed personalized care plans, including Lifesong-organized therapies.

My Lifesong Music Therapy is a program customized to the needs of our hospice patients to improve their quality of life while on the hospice journey. Music Therapy can shape behavior and brain function as well as reduce stress and allow patients to connect with others.

Celebrating Legacy

We start every care plan in learning about the patient. Not just their medical needs, but importantly, what brings them joy. In understanding what they loved, where they traveled, how their careers unfolded, what hobbies and pastimes they enjoyed most, our team learns about the patients and can provide personalized care and plan celebrations tailored to them that family and friends can participate in to honor their loved one.

With My Legacy Projects, we work with families to creatively fulfill an activity, organize a collection or create an experience that leaves the patient and participants with warm memories as they celebrate with their loved one.

Preserving Dignity

Hospice and palliative care are about comfort and quality of life while living with a chronic illness or at the end of life. Physical care is important, so is taking care of the spiritual and emotional needs of the patient and their family. Our goal is to ensure that our patients are living each day with grace.
Our name: Lifesong. In everything we do, we honor the unique lives of our patients—their Lifesong. Every patient and family are different; therefore, we do not take a cookie-cutter approach to hospice and palliative care. Life is something to be celebrated. We hope our personalized care and attention brings comfort and peace during this emotional time.
